I was given a form for omma in oklahoma…
I was given a form for omma in oklahoma and it has a new york doctor and address on it. That's not allowed in oklahoma. It also has the name of a doctor that is on trial for fraud paperwork and a $35 million dollar scheme. Dr Joel durinka. It also doesn't list any reason on why it's being recommended.

You can visit the Oklahoma Medical Board website and verify the doctors medical license is active, in good standing with no disciplinary taken against them.
The physicians address on the form is where they receive mail which is needed in case the Oklahoma Medical Marijuana Authority (OMMA) needs to get in touch with them. This address must match what the Oklahoma Medical Board has on file which you can see when you search their license on the state website that the addresses match. It is very common in telemedicine for a physician that is licensed in multiple states to have a single address where they receive all of their mail. This is why you see an out-of-state address on the form. Please search their license on the Oklahoma Medical Board website and you will see the same address listed.
The reason there is no medical condition listed on the medical cannabis certificate is because this field is Optional. The state made this optional for patient privacy because patients are required to carry this in case they need to prove they are a registered medical cannabis patient. Most patients prefer to keep their medical condition private which Oklahoma allows. For patient privacy most doctors do not complete this Optional field. If you would like the physician to enter your medical condition on your state form please let us know.
Alternatively, if you would prefer, the doctor can send a revocation of license form to OMMA, revoking the certificate that was issued, which will allow you to get certified by another physician.
