Safe Transitions Care is an independent organisation established to provide targeted life opportunities to vulnerable young people to enhance their prospects of living as independent a life as they possibly can within an enabling and homely supported environment. The primary purpose of Safe Transitions Care is to provide leaving care support for young people that have left care. Our support is versatile as it incorporates young people with mild to moderate emotional, behavior and learning difficulties, consistent with the ‘Framework for the Assessment of children In Need and their Families’ documentation. We provide support to vulnerable young people both male or female aged between 16 and 18 years old (or 21 years if in full time education) and our client group entails: • Young People who are vulnerable and with complex social/emotional needs. • Young People who have challenging behaviors, a history of offending behaviors and may have involvement with the criminal justice system like the Youth Offending Service (YOS) and children and adolescent mental health services (CAMHS). • Care Leavers who are subject to Pathway Plans. • Unaccompanied Asylum Seeking Children (UASC). • Young People at risk of Child Sexual Exploitation (CSE) and Child Criminal Exploitation (CCE).See more
