Paid for nothing
I applied for my marriage license in April and it is now July 22. I got this notice for my feedback, but I have not received my certificate. I paid $89.00 for nothing! Very disappointed!

I sent the information you sent me to sign and date. It was returned to me from the post office saying the postage said a package and it didn’t meet the weight requirements. This is crazy you charging me $89.00 for what? I want a refund ASAP!!!!!!

The process required by the Health Department to obtain a certificate requires a study of the application but in many cases, the health department restricts certain requests for personal matters.
These processes are internal to the health department and we have no control over them.
When they returned your request, they should have attached a document with the reasons why your request was returned. Customer satisfaction is our number one priority. We have provided you a full refund since we can see you are not satisfied. We hope this will resolve the issue and clear the air of any misunderstanding.
Still haven’t received my sons birth…
Still haven’t received my sons birth certificate. I think my son may have made a mistake when sending back the packet, because the envelope that the new birth certificate was supposed to be shipped to my address came back empty. I feel like I should have received it and even with this minor mistake, I feel like I should have received something in the mail telling me how to go about getting it. Or you guys should provide your own envelopes to send out the certificate if this mistake was ever made. Not satisfied at all feel like I waisted my money and my son still doesn’t have it and still can’t get his learner’s permit or even state I.D card. Please either refund my money or send me a new packet or something!

Process was good. Could have used a little better clarification on the difference of paying. It cost way more to do paper application rather then we could have printed it off for cheaper. Oh well live and learn

Been over a month and still have no birth certificate. No records at Vital Statistics about the order to obtain my birth certificate. Sent money to you and got a piece of paper to fill out and envelopes, but now nothing. Thanks for stealing my money. Also reported to BBB and post negative posting online.

What started out a little complicated…
What started out a little complicated with questions turned into a quicker road then thaught. Just make sure you do the paperwork slowly with not a single error in spelling or boxes not checked. Make sure if you are older trying to get a passport you get a new birth certificate. The original one that I have does not have my parents information allow extra time. It would be nice if the passports are connected with government issued paperwork like your ss # and expecially your birth certificate. Now I’m praying I will get my paperwork to get on a well deserved vacation

I received the packet back saying it didn't have all of the information even though I filled out everything you guys marked as needing. All your information was wrong. Luckily I knew someone that worked at the Health Department that you had me send all my documents. She helped me out and I received the certificate the same day. You even had me.semd.the wrong amount.
