Peace of mind!
After a few glitches and frustrations I used the much easier process of simply giving the info to Passport Visa Express. They could size my photo and passport page and fill in all the foreign locations and in less than a week I received a copy of approved visa to download. I would use this service again

More explanation about my question about a Laos Visa instead of a QR Code may have been helpful. I was also not instructed well about pictures. I ultumately had to go to a camera company for passport pictures after sending the selfie to you from UPS, only to be told the size was wrong. I spent much time and money on this.
