I had some difficulty in Finding & answering the Questions on Line
I had some difficulty in Finding & filling in the correct form. I would suggest
that the information you need for placing an order should clearly say so and have only one page to to fill in.
I had to ask Lex to assist me, because I had filled in about three pages
which was not seen.
